From checklistto verified fix
You plan the audit, run it, write the findings and come back to check the fix. Orisynx keeps each of those steps linked, from the first checklist answer to the effectiveness review.

The same checklist on every site
Start an audit from a template and work through the same questions, in the same order, wherever you are auditing. Each answer is recorded as in order or as an exception.
An audit moves from draft to planning, active and completed (or cancelled), so its status is always clear.

Findings that stand up
A finding is linked to the audit it came from and graded low, medium, high or critical. Start it as a draft while you check the facts, then open it when it is ready for the auditee.
Comments keep the discussion with the finding, rather than in an email that nobody can find at the next audit.
- Finding statuses: draft, open, in review, closed
- Severity: low, medium, high, critical
- Comments on the record
Approved supplier list not reviewed since March 2024
Three of eleven active suppliers have no evaluation on file.
Evidence attached where it belongs
Attach files, links and notes as evidence: the photograph from the shop floor, the record you sampled, the link to the procedure. Evidence stays with the record it supports.
Controlled documents live in a separate document library, so the evidence you collect is kept apart from the documents you audit against.

Verify before it closes
Corrective actions move through open, in progress and verify before they close. At the verify step, record whether the action was effective, partially effective or ineffective, or leave it pending until there is enough evidence to judge.
Where a site visit is needed to check, plan a follow-up audit: it is one of the audit types in the programme.
Internal auditors: questions
Can I record findings as major or minor nonconformities?
Findings are graded low, medium, high or critical. Many teams map their major and minor scheme onto those grades and write the mapping into their audit procedure.
Does Orisynx come with ready-made checklists?
No. Templates are built by your organisation and can follow the clause structure of the standard. Once built, they are reused for every audit of that kind.
What if the auditee disagrees with an action?
The person responsible for an action accepts it or disputes it with a reason. The dispute and the discussion that follows stay on the action.
